Please download the form above and return the completed form via e-mail with a digital photograph to Allison@maoteen.org. Passes will be available for pick-up at the Rosen Centre Miss America's Outstanding Teen Information desk. Check the MAOT website for operating hours.

Two forms of I.D. will be required at check-In along with a business card or a card from your publisher/editor.
